DUE DATE FOR REPORT / LOGBOOK/ CD SUBMISSION IS 31 JANUARY 2011 at
SIIU office from 8.30am to 4.30 pm.

YOU ARE REQUIRED TO SUBMIT THE STUDENT EVALUATION FORM (form ITC- G)
TOGETHER WITH THE REPORT (please refer Internship Guidelines,
Appendix XII , page 45).

PROCESS FLOW FOR REPORT SUBMISSION:

1. STUDENT TO SUBMIT COMPLETED ITC-G FORM

2. SIIU STAFF TO STAMP RECEIVED DATE ON REPORT/ LOGBOOK AND RECORD
ALL THE ITEMS RECEIVED.

3.STUDENT TO SUBMIT REPORT/ LOGBOOK/ CD TO VISITING LECTURER

2. We’ll print it for you (20 cent/page)

3. Hardbound completed in just two days
